Iron Infusions vs. Supplements: Which Works Faster?

My Personal Battle With Low Ferritin

I've been right where you're sitting, feeling completely drained and wondering why my body wasn't cooperating.

When my ferritin dropped to 14, it showed up as rabid (yes, I'm choosing the word rabid over rapid on purpose) heart palpitations that were very distressing.

I spent six months diligently taking 150 mg of oral iron every other day, only to watch my ferritin crawl up to a measly 36.

If I missed taking my iron for even a few days, those terrifying palpitations came roaring right back.

My energy was completely shot, and my brain just wasn't turning back online the way I expected. Why did I come into this room?

At this point I decided to learn how to provide iron infusions, using myself as a guinea pig. I gave myself four weekly iron infusions. You can check out my experiences with each at the bottom of this article.

After my next blood test, my ferritin reached a vibrant 109.

The difference was night and day!

My energy was full of vitality, my brain felt alive, my exercise endurance shot up, and my heavy periods became significantly lighter. It's an amazing feedback loop: when women have adequate iron, they actually lose less blood during their cycles.

Experiencing that incredible transformation firsthand inspired me to help others get their iron levels right where they belong. I'm VERY passionate about this now. A friend / colleague of mine who also offers iron infusions and I say to each other often that we feel like we're saving the world one iron IV at a time.

Why Oral Iron Supplements Take Months (And Often Fail)

If you've been swallowing iron pills for months with zero results, you're definitely not alone. Absorbing iron from food or supplements requires a very specific digestive environment that most people in our modern world simply don't have.

When we're stressed and tense, our nervous system can't shift into parasympathetic mode, our essential "rest and digest" state. Without that, our bodies don't produce enough stomach acid to dissolve oral iron and make it bioavailable.

Getting supplements to absorb properly becomes a delicate dance of timing that cat-like precision, and frankly the modern odds aren't in our favour:

  • Dietary Inhibitors: Caffeine (nature's cortisol) blocks absorption, along with calcium, phytates in grains or seeds, and oxalates in certain veggies.

  • Gut Distress: Oral iron is notoriously harsh on the digestive tract, causing severe abdominal pain, constipation, bloating, and gas that many people can't tolerate.

  • Aging and Enzymes: As we get older, natural digestive enzyme production slows down, making absorption even harder.

  • Low Daily Yield: Average absorption rates mean you're likely only absorbing a tiny 1 to 2 mg of iron per day from pills.

Iron Infusions vs. Oral Iron: The Math Behind the Speed

So, which option actually works faster? It isn't even a close race. IV iron laps oral supplementation several.times.over.

When you take oral supplements, you're looking at a multi-month slog of guessing, digestive discomfort, and minimal gains.

With intravenous therapy, we can estimate that every 100 mg of iron administered directly into your bloodstream raises your ferritin by roughly 12.5 points. Now we have a clear, attainable target.

That means in just four-five short weeks of weekly treatments, you can completely restore your iron stores and jump from feeling exhausted to vibrant.

For my patients, results happen fast. Most people notice a major energy boost that afternoon or the very next morning. I can't tell you how often I hear patients say, "I woke up before my alarm for the first time in years," Or "I woke up without hitting snooze once!"

Understanding Your Lab Results: The Peanut Butter Shelf Analogy

Determining whether you're a candidate for an infusion is straightforward. If your ferritin is under 100 ug/L, you're eligible. Most patients come see me when their ferritin has dropped below 15.

More and more now I'm hearing patients tell me that their family docs suggested for them to get private infusions because their levels are low, but not low enough for hospital treatment, covered by MSP.

To understand why you're feeling so terrible even if your doctor says your hemoglobin blood count looks "normal," picture a grocery store shelf:

The Peanut Butter Analogy

  • Hemoglobin (The Shelf): Think of hemoglobin as the eight jars of peanut butter on the store shelf. Every time you buy one, the store restocks it from the back storage room so the shelf always looks full.

  • Ferritin (The Back Storage Room): When supplies stop coming in, workers keep pulling jars from the back room until it's completely empty. Only when the back room runs out will you finally notice missing jars on the shelf.

Your ferritin (storage) drains long before your hemoglobin (blood count) drops! You can be profoundly iron deficient without being fully anemic yet.

If you have chronic inflammation, we look deeper with a full iron panel including C-reactive protein (CRP) and transferrin saturation. If CRP is elevated, and your transferrin saturation is less that 20% you're eligible. A ferritin reading of 99 might only represent about 33 ug/L of usable iron, making an infusion essential.

Are Iron Infusions Safe?

It's completely normal to have questions about side effects, but real-world risks are exceptionally rare.

I use iron sucrose (Venofer), where the risk of a severe allergic reaction or anaphylaxis is just 24 in 10,000—that's a tiny 0.24% chance! I keep a fully stocked emergency medicine kit with epinephrine right on hand for ultimate peace of mind. Out of all my patients:

  • Zero patients have reported skin reactions.

  • Only one patient ever reported a mild headache after her first session.

  • About 1 in 10 patients feels a bit tired right after their infusion.

That post-infusion fatigue is simply your body getting access to a vital nutrient it's been starved of for months. It wants to rest so it can use that iron to heal. By the next morning, patients perk right up.
